Terms & Conditions:

1) Puppy must be less than 12 months of age
2) A fully completed application form and AVS (formerly known as AVA) License Certificate (with microchip number indicated on AVS License Certificate)
3) If any information is not provided, we reserve the right to reject or delay the application until all information is completely given
4) We will process your application form within 14 working days upon receipt of complete and accurate submission of all required documents
5) PLC reserves the rights to change these terms and conditions, change or substitute products without notice

What we need from you:
1) Fill in the form given below
2) Attach a photocopy of your "Puppy AVS Licence Certificate"
3) The Microchip number indicated on the AVS certificate

Note: NRIC/FIN is NOT required. For submission of AVS license, please conceal/cover your NRIC/FIN.
